This notice describes browsing the website for Van Metre Chiropractic and using its contact form. It is separate from the practice's clinical privacy notices and patient-record procedures.
Website information
Squarespace hosts this website. When you visit, your browser sends technical information needed to request and display pages, such as your IP address, browser or device information, requested pages and request times. Squarespace processes website usage information for hosting, security and other purposes explained in its Privacy Policy.
The contact form requires your first name, last name, email address, phone number and Chief Complaint, a brief description of the reason for your visit. These details help the office understand your request and follow up with you. Your Chief Complaint may include health information. Squarespace processes your submission and stores contact information and form submissions in its Contacts area. The form is configured to send an email notification containing your submission, including your Chief Complaint, to the office at office@vmchiro.com. Email and office systems are separate from the website's storage.
Submitting the form does not confirm an appointment. Please keep your Chief Complaint brief; do not include your full medical history, insurance information or medical records. Call 623-209-2225 to ask how to provide further information for your care. For a medical emergency, call 911.
This website does not provide patient uploads, patient accounts, online booking or payment entry. You can also use the telephone links to call the office. Calling involves separate telephone and office systems.
Cookies and external services
Squarespace's nonessential cookies are restricted, and its individual-visitor activity log is disabled. These settings do not eliminate all technical collection, platform measurements or third-party technologies.
The embedded Google Map connects to Google when it loads. Google may receive technical information such as your IP address and request details, and may use cookies or information associated with your browser or Google account. Other external resources, including fonts and scripts, also require requests to their delivery providers. See Google's Privacy Policy.
The contact form also uses Google reCAPTCHA to help protect against spam and automated submissions. Review Google's Privacy Policy and Terms of Service for information about Google's services. The Squarespace cookie settings described above do not establish that reCAPTCHA or other Google services are blocked.
You can manage cookies through your browser and review your Google account privacy settings. Blocking technologies may affect website features. The linked provider policies explain their own processing, retention and available choices.
